AIA Gateway doesn't respect YOLO config and constantly request permission

Unity AIA Gateway (with Gemini in the exact test case)

Steps to reproduce:

1. Open AIA, choose YOLO mode.
2. Enable Gemini Agent
3. Execute a prompt that could read/write/create files.

Actual results: AIA constantly ask for permission

Expected results: AIA automatically execute commands. YOLO!
