Unity Pipeline server can go down from a Codex call

Bug repro 

1. Open a Unity project that has Pipeline installed.
2. Open Codex CLI and set Model Permissions (/permissions) to "Ask for approval" or "Approve for me" so that the sandbox is enabled.
3. Ask Codex to connect to the Editor via Pipeline.
4. It fails.
