Breadcrumbs don't change names when graphs are renamed

Steps to reproduce:

1. Open the "65ETS.zip" project
2. Open the "New Graph" graph
3. Enter the subgraph "New Graph 1" from within the canvas
4. Enter the subgraph" New Graph" from within the canvas
5. Repeat step 3
6. Rename the "New Graph" node to "Test" in the Graph Inspector
7. Observe the breadcrumbs

Actual results: Only later accessed "New Graph" breadcrumbs open with name "Test"

Expected results: All "New Graph" breadcrumbs change title to "Test"

Reproducible with versions: 0.4.0-exp.2 (6000.3.0a6, 6000.3.15f1), 6000.4.7f1, 6000.5.0b8, 6000.6.0a4

Can’t test with versions: 6000.0.75f1 (No Graph Toolkit)

Tested on (OS): Windows 11 Pro (25H2)

• Only newly accessed instances of breadcrumbs have the new name
